Text Box: Treatment of patients 65 years of age and older. Knights Templar Eye Foundation is a partner with EyeCare American Seniors EyeCare Program, a public service foundation of the American Academy of Ophthalmology. Through this program, any eligible senior is referred to a nearby volunteer ophthalmologist for eye care at no out-of-pocket expense. The Seniors EyeCare Program (SEP) is designed for people who are US Citizens or legal residents and are 65 years of age or older, do not have eye care insurance through an HMO or the VA and have not seen an ophthalmologist in three or more years.
Seniors may call the Seniors EyeCare Program toll-free helpline at 1-800-222-3937 which functions 24 hours a day, seven days a week.
A special provision, waiving the three year limitation is part of the agreement through another toll-free administration number 1-877-887-6324. This is contingent on any surgery not having been schedule and the physician agrees to see the patient as a volunteer under the program. This is an opportunity for the foundation to obtain care for our age 65 and older patients, without having an outlay of funds from the Knights Templar Eye Foundation to the ophthalmologist. The foundation can still provide incidental hospital and anesthesiologist expenses related to surgery for those who qualify under our current guidelines. This can greatly extend the ability of the foundation to serve patients in need of assistance. Applicants must provide a copy of their birth Certificate. Legal residents must provide a “voided” copy of the Naturalization Certificate. All applications must be on original forms with original signatures. This article was written based on information in correspondence dated August 25, 2003 and November 24, 2003.
